Position Description

Empath Health is a nonprofit integrated network of care that provides expert, life-changing health care to those with chronic and advanced illnesses. Empath Health includes hospice, palliative home health and physician services, independent living assistance and day care for seniors, HIV services and advance care planning.

Suncoast PACE (Program of All-Inclusive Care for the Elderly) is a nonprofit provider of comprehensive health care and support services to Pinellas County seniors with chronic health conditions. As a member of Empath Health, our focus is on helping participants remain healthy and independent while living in their own homes.

Under the supervision of the PACE Center Manager the Dietician/Life Style Coach manages the Healthy Living and Diabetes Prevention Grant (HLDPG) funded under a three year contract. This is an exempt position with a 32 hour work week. The Life Style Coach uses pertinent data to plan and implement the HLDPG designed to provide additional opportunities for PACE participants to have access to healthy food, received meal preparation instruction and participate in health and wellness coaching and physical activities. The Life Style Coach will oversee the HLDPG and ensure successful implementation of said grant.

The Lifestyle Coach will conduct nutritional assessments, assist in determining PACE participants to participate in Grant and coordinate interventions, with the interdisciplinary team to create a comprehensive plan of care appropriate for the grant goals and purposes. The Lifestyle Coach will also track and record data for the Grant and document pertinent activities and data in the medical record according to PACE regulations and Empath standards. The Lifestyle Coach will work with the RD , Occupational Therapist and Speech Therapist to ensure that Grant participant meals meet the standards of the Grant and PACE regulations. The Lifestyle Coach develops and provides diabetes and nutritional educational programs to Grant participants and implements nutritional components of Grant Program such as developing therapeutic dining programs, food assistance, food preparation and food acquisition.

Position Requirements

As a Dietitian Lifestyle Coach with Empath Health you must be patient/family focused, loyal, motivated and interested in serving your community.

Other requirements of this role include:

Education: High School Diploma (with prior Grant experience) or Bachelor's or Advanced degrees from an accredited college.
Experience: Minimum one year experience working with a Grant or working with the frail elderly population.
Valid CPR certification from the American Red Cross or American Heart Association.
Dietary Experience a Must with teaching and educational experience given preference.
Must have a valid driver's license, insurance and means of transportation
